In consideration of your acceptance of my entry, I hereby waive, release and forever discharge any & all claims for damages which I may have, or may hereafter accrue, against USATF, Lone Star PV, LLC, other organizations providing volunteers, or other sponsors for any damages which may be suffered by me. I certify that I am in good health and that my level of training is such that I am prepared to compete in this event. I also give permission for media created at this meet to be used for Lonestar PV promotional purposes. A parent or guardian must sign this form to allow a minor to participate.
